Another possibility is that a person does not have enough folding around the edge of the ears, or the antihelix. If this is the case, the cosmetic surgeon will reshape the antihelix by scratching the front of the ear and folding it over and pinning it from the back.

Occasionally, the surgeon will find the need to make a small cut on the front of the ear. In this situation, the surgeon will make the cut in a fold or crease so that it is effectively hidden.
