Ancient Surgeries Lead To Today's Rhinoplasty

Rhinoplasty is a plastic surgery procedure meant to reconstruction or enhances the nose. There are a wide range of reasons for the procedure as well as many different methods to perform it. We believe these surgeries to be a modern day idea, but their roots and many of the techniques go back to ancient times.

The first recorded nose surgery was recorded by physician Sushruta around 800 BC. He intended his work to repair damages caused by religious, criminal and military punishments. His development of forehead flap rhinoplasty is still used today to repair nose trauma.

No new developments were made until around 300 AD when royal doctor Oribasius described a new surgery meant to help disease victims in the Roman Empire. His new methods allowed facial reconstruction to be done without distorting any other facial flesh. Instead he would remove skin along with its blood supply from a non exposed part of the body.

In Dark Ages the world of rhinoplasty finally became part of Western culture with the Anglo-Saxon physician's manual called Bald's Leechbook. The manual described in great detail the Asian techniques for repairing cleft lips as well as other forms of nose surgery. By the 11th century the Asian techniques had also been translated into Arabic.

In the 16th century a professor of surgery and anatomy at the University of Bologna named Gasparo Tagliacozzi published his new technique for reattaching a nose using the bicep muscle pedicle flap. This was the first time muscles had been used in a reconstruction.

By the 18th century improvements in Indian skin flap techniques had developed considerably. Reconstructed noses were made more natural looking and there was less scaring on the forehead where the tissue is removed. This was done by fitting the nose with a wax cast to insure proper sizing.

Johann Friedrich Dieffenbach was one of the first surgeons to anesthetize a patient before performing a nose surgery. This new 19th century technique allowed the surgeons to take their time and refine their cuts for more precise and accurate techniques. In 1887 the first closed or endonasal rhinoplasty was developed in the United States by John Orlando Roe. The new method was a revolutionary approach to repairing deformities.

The early 20th century gave birth to the submucous resection septoplasty with the work of doctors Freer and Killian. This new discovery was the first of many improvements within the century. Peer and Metzenbaum performed the first manipulation of the caudal septumin in 1929. In 1947, Maurice H. Cottle improved septal deviation surgery. Closed surgeries were the norm until the 1970s, when Padovan advocated the open approach with new techniques. With new techniques more and more patients were willing to go under the knife in an effort to improve their appearance.

The latest advancements have been in the area of injection soft tissue fillers for non surgical plastic surgery. These fillers can correct deformities and defects in a less intrusive way which lead to shorter healing times. Rhinoplasty has come a long way since the first Indian skin flap techniques. Today's surgeries are far safer, less painful and less intrusive then every before.
