Heart disease is the single biggest killer of people worldwide, with all the different varieties of cardiovascular disease accounting for nearly 30% of deaths every year. It seems that the lure of the security that easy food brings is too much for many to resist. Each extra unit of Body Mass Index (BMI) over the recommended range was associated with a 5% increase in the risk of heart disease for women, and a 7% increase for men. There has been much publicity about heart healthy diets recently, and Thai medical centers have been acting on the most recently available research - advising patients to cut down on saturated and trans fats, reduce sodium intake, and opt for whole grains over refined wheat products in order to avoid undergoing cardiac surgery in Thai hospital heart centers. However, the reality of what is needed to reduce this risk at first may be much simpler than scientists previously thought, and the new advice may have the potential to save many more lives.

The new advice that international hospitals in Bangkok are giving their patients is simple - just eat less. In a very new study, total calorie intake was shown to be more important to achieving and maintaining weight loss that worrying about differing proportions of fat, protein and carbohydrate. Diets like Atkins, and others that focus more on proportions of different foods than total calories don't necessarily work as well at keeping people off cardiac surgery tables as simple calorie restriction does.

The study involved 811 overweight and obese adults aged between 30 and 70. Each adult was assigned one of four diets, and asked to record their daily intake of different foods in a diary. They also attended bi-monthly diet counseling sessions, and had individualized calorie goals per day, with the target set lower than their total calculated requirements. Study participants on all of the different diets showed similar reduction in risk factors for heart disease, such as LDL levels, lower blood pressure, reduced triglycerides, and higher HDL (good) cholesterol levels.

For hospitals in Bangkok, this makes prevention and cure of heart disease much simpler. In the beginning, all that needs to be done is to reduce the total number of calories consumed by a person to a number beneath their weight-maintenance requirement. Later, variety and nutrients are important, obviously (good health is not sustainable on a two-chocolate bar per day, and nothing else, diet). For patients at Thai medical centers who have expressed concern about understanding what is healthy and what is not, though, this finding has great potential.

However, the study did move much more slowly than many doctors at international hospitals in Bangkok would recommend for their patients. Some patients at Bangkok Hospital need to lose weight much more quickly than the 10% of body weight over 2 years that the study achieved for 15% of participants. One factor which can assist in exercise, naturally. Only 90 minutes of exercise per week was asked for in the study, but this can be increased dramatically for many individuals (in consultation with Thai heart centers).
